Discharge planning is a crucial aspect of patient care. These nurses help to ensure that the same quality and frequency of medical care is provided in order to prevent re-hospitalization. The discharge nurse is the liaison between the patient and other health care providers in the community.
So what exactly do the discharge planning nurses do? The following are several of their duties:
- Establishes patient referrals with other health services such as physical therapy or finding a physician.
- Initiates discharge planning procedures. They work with the physician and the patient’s nurse to be certain that all plans are in order for the time of discharge. They verify that the patient and the family have an understanding of these.
- Initiates specific care plans with the patient’s family.
- Coordinating teaching for the patient and the family.
- The discharge nurses verify that the patient has the devices needed for home and verify insurance coverage.
Discharge Planning nurses will work with you and your family-along with your health care team to assist with coordinating your insurance benefits and health care needs to prepare you for discharge.

Food and Nutrition Services
We provide high quality food, outstanding service, and satisfaction to our customers.
Our inpatients meet daily with a hostess to plan their meals and ensure that all nutritional and personal needs are met. Our dietician will make sure that, if a patient has special needs, they receive the materials and information necessary to maintain their nutritional health even after going home.
The Cascade Cafe is a retreat for family, guests, and employees. The Cafe is open to the public and welcomes all guests to a great meal for a great deal. We offer full breakfast, lunch and dinner menus on weekdays and a limited weekend and holiday menus. Our specials, entrees, grill and deli choices, homemade soup, and Grab-N-Go items offer something for everyone. Our current menu can be found here.

Gifts and Espresso
Located in the main lobby next to the fireplace, Cascade Valley Hospital Gifts and Espresso offers a great selection of fine gifts, treats and specialty coffee drinks. Hours are Monday-Friday, 8:00am-4:30pm and Saturday, 10:30am-3:00pm. For more information, please click here.
When you need a copy of your medical record, we will be happy to provide it upon receipt of your signed release. You will need to show photo identification. You may also download and complete a Medical Information Release form, found here, and fax it to 360-435-0525.
Washington State law sets the fees charged for copies of medical records. There is, however, no charge for sending your records to your physician or health care provider or another medical facility.
If you do not need copies, but wish to view your record, you can call the Health Information Management department at 360-435-0502 and schedule a time to do so. Office hours are 8:30am – 4:00pm, Monday – Friday.
Birth and death certificates are not held in Cascade Valley Hospital records. To obtain an official copy, contact the Snohomish County Health Department at 425-339-5280.
Chaplains are very special volunteers at Cascade Valley Hospital. They are pastors of local congregations who have training and experience helping individuals and families in times of illness, surgery, or personal crises.
If you or your family need someone to talk to, someone who will listen, or someone to help sort out information in making ethical decisions about health care, please let your nurse or doctor know and they will contact a chaplain.
When you have a question, complaint, or compliment, our Patient Advocate is here to help. No issue is too big or small, whether it’s a care concern or a process question.
The Patient Advocate is available 8:30 a.m. – 5:00 p.m. Monday through Friday.

Patient Financial Services
As much as possible, we want to reduce the financial worry of needing the services of Cascade Valley Hospital. Our financial services team can help you understand the billing and insurance process and help you with other forms of payment.
You may pay your bill with cash, check, Visa, Discover, Mastercard, or American Express. We can also help you apply for financial assistance. If you do not qualify for the assistance program, we offer a 12 month interest-free payment plan.
If you need financial assistance information, or have questions about your bill, please call 360-618-7620 from 8:00 a.m. – 5:00 p.m.
You will be treated in the Emergency Department at any time, regardless of your ability to pay. Please do not hesitate to seek the care you need.

From greeting visitors at the front desk, to assisting in the emergency department, our volunteers are able to experience many different aspects of our hospital.
