Cascade Valley Hospital ranked third among 48 hospitals in Washington state for 30-day Medicare readmission rates in 2016. Cascade Valley Hospital’s rate of 11.6 percent is well below the state average of 15.2 percent and the national average of 18.4 percent.
Medicare readmissions have steadily improved at Cascade Valley Hospital since 2014 when the rate was 15.2 percent.
Staff and providers have put a great deal of work into reducing readmissions including putting a focus on communications around patient discharges, process improvement and multidisciplinary rounding. In addition, starting in 2014, a quarterly “Smooth Transitions” meeting is held including hospital staff and representatives from community providers of care such as skilled nursing facilities and home health, to work on communications and improving patient transitions from the hospital to other care settings.
